Hmm, I just noticed you've actually used that method. No idea why you get that result.
Check your spline, if they are all on the same plane. (all the vertexes should be flat on the Y-axis in your scene)

Looks like you need to manage your cuts to the outter shape.
I have had this in the past with multiple shapes that I attached then extruded. It has to have a cut from the hole to the border, best to make your own, that are as direct as possible, then remove the edge it puts in for ya.

EDIT :just did a quick test, made multiple shapes and attached em all then extruded, max didn't put in any cut lines from the shapes to the outside. I believe this is because max can be frickin odd sometimes, I have often used this method to produce custom shapes and have always had to replace the cuts from the holes to the border to get it clean. This time , nope, max doesn't need it, go figure. Also max sometimes has days where it wont show anything in the sub object selection menu. For no reason! or where it wont snap to verts ! sigh.

Be a lot easier to help if max was consistent!

The reason I didn't see my cut lines is because I didn't convert to an E-poly , once you do that you see the cuts. I'll do a quick vid for you on cutting, it is a noobish question but thats ok . You should do some tutorials :)

Try extrude instead of shell.

I used the extruder instead of the shell, but it chose to do this:
And it supposed to be filled, which it isn't.. I have no clue what's going on..
I also tried to cut like show in the video by Davious (Still have no idea why you are supposed to do that) but when I used the cut and cutted like shown in the video and deleted the lines going to the corners it deleted a whole section.. It's really annoying that the first time i'm trying to shape a model it comes bite me in the a** :P

Mc, that image looks like you need to weld verts. I could do another jing for you from start to finish but you would't learn .

Select your object , go into sub object, vert mode, then CTRL-A to select all, click the dialog windon next to weld, there may be a difference in the before and after numbers.

Click weld if there is.
