I want to create a "work request form" that a client fills out with their details etc. in order for me to start the job. But I am sure there's an actual name term for it.

Usually. But why would you be prepared to start work if you don't know how much you'll be paid, or if the client doesn't know what their bill will be?

hmm, I think I want something with no costings its more of a please supply your name address and work required. Then I can go back with an estimate of cost and hours.

im used to them being work orders, my company calls them purchase orders.

you choose. no right or wrong rule. working for yourself you'll find these 'officialties' often start to get ignored and not used ;)

Yeah I just want a form that gives me their name, possible budget, completion date etc so I can put it on record.

I was going to call it "work request form" A PO looks like its to much to fill out in the early stages
