Terrain program / Image type / techniques ?

Hi all, need help on the issue of modelling terrain. usually i tidy up data in autocad then import, thern convert to editable mesh, which leaves a messy mesh. I know I've come across a few people here who have a more professional approach to terrain, maybe using other software. I'm wondering about the using height data images then somehow converting to a mesh if possible. We need to be able to cut in roads etc, so a mesh is needed at the end, unkess there is some accurate way of manipulating height data images.

I need as much info as I can get at the mo, so fire away even if it seems obvious. Cheers folks, any help will be very appreciated. Cheers.

displace modifier, then flatten the modifier stock?
