Help with animation problems please!

I have been modding character meshes in Mass Effect 3. Most have worked fine but I have one that is causing problems and I don't know what the issue is or how to fix it. Here is a side-view of the original character mesh:

I corrected the posture and made a few minor adjustments to other parts of the body but nothing major. No vertices added or subtracted:

I've done this twice by two different methods but the result is the same. When she is animated in the game and she turns her head, her eyes bulge out through her eyelids a little bit. The first method I used was to select all vertices from the waist on down and shift that part of her body forward, then blended/smoothed the border areas together. The second time I selected all the bones from her lower back on up, arms inclusive, and shifted them backwards slightly, no smoothing required.

How do I fix this? What is the issue?

how are the eyes linked? It almost looks like there are multiple position constraint targets and its blending between them or something... maybe you can just re-link/constrain them to the head? or grab all the head verts and give them more weight into the head

is she toothless? thats creepy

She's not toothless, but her teeth appear pink(ish) in the scene...in hospital bed, injured.

How would I re-link/constrain them to the head? I'm new to 3ds coming from Blender.

There's also issues with how her hair lay around her shoulders...like its floating above them.

I was just futzing with the character in 3ds and noted that the eye bones are located immediately in front of each eyeball and the pivot point for each eye is co-located with the bone in front of the eyeballs. How do I adjust the pivot point/bone position so they are centered in each eyeball? Every time I try to "Affect Pivot Only" on the bone it nevertheless moves the eyeball so it is always in the "wrong" position.
